Build Behaviours. Build Bones

A structured framework for integrating behaviour, tissue adaptation, and metabolic health into everyday clinical care - and into real lives.

Founder

Dr Ruth White

I’m Dr Ruth White PhD, founder of Build Behaviours Build Bones. I am a pain physiotherapist and behavioural science researcher.

My clinical work focused on complex, persistent pain presentations and whole-person systems. My PhD deepened that translational lens - how behaviour, biology, and environment intersect in real-world care.

I also live with low bone density.

That experience shifted something for me.

Bone change is slow.

Fear shrinks lives quickly.

What I saw both clinically and personally, was that people were finding it challenging because plans were too complex, too frightening, or too hard to repeat once supervision ended.

BBBB extends my translational work into behaviourally informed, metabolically aware skeletal care.

It is structured. It is calm. And it is built for longevity.

Bone health is not a niche interest.

If you prescribe load, you influence tissue adaptation.

Build Behaviours, Build Bones (BBBB) brings together behaviour, osteogenic load, and metabolic context into a clear, repeatable structure that works in real clinics and real weeks.

Bone is a visible expression of adaptive capacity — not an isolated specialty.

Tissue does not adapt in isolation.

• Behaviour determines what actually happens between sessions

• Mechanical load provides the stimulus

• Metabolic health and recovery influence resilience

• Environment determines whether the plan is repeatable

Clear enough to teach.

Safe enough to repeat.

Simple enough to defend.

Course Description

  • Path 1 for Women

Strong bones are built through repeatable signals — not panic and not perfection.

This course is designed for postmenopausal women because skeletal fragility is most visible in midlife and beyond.

But the principles that underpin it — load, nourishment, recovery, and behaviour — apply across the lifespan.

    Adaptive capacity is not age-bound.

    What you receive

    • A clear weekly structure

    • Two protected anchor sessions most weeks

    • A Safe Base for lower-capacity weeks

    • Simple progression rules

    • Calm explanations that reduce fear

    • Guidance around nourishment and recovery support

You don’t need intensity for its own sake.

You need a plan that fits your real week and can be repeated long enough to matter.

    Bone adapts slowly.

    Consistency wins.

  • Path 2 For Clinicians

A structured clinical reasoning framework for bone adaptation and metabolic context.

The BONES framework helps physiotherapists and exercise physiologists integrate bone health into everyday care.

    Osteoporosis represents constrained adaptive capacity.

Bone adapts when behaviour, load, metabolic context, and environment align.

Most clinicians were not trained to assess those layers systematically.

    The BONES Framework

    At the centre: Behaviour

    (capability, opportunity, motivation)

    Surrounding layers:

    B — Behaviour

    O — Osteogenic load

    N — Nourishment (metabolic context)

    E — Environment

    S — System structures

This is not a protocol. It is a way of thinking.

    For Clinicians Who Want To:

       •   Integrate bone literacy into mainstream practice

       •   Apply osteogenic load safely and confidently

       •   Understand metabolic influences on adaptation

       •   Support behaviour change without overcomplicating care

    Portable. Structured. Lifespan-relevant.

    Become bone-literate in everyday practice.
